BreastScreen Rockhampton LGBTIQ+ Advisory Group Health Consumer Representative Application form

Closed 31 Mar 2021

Opened 1 Mar 2021

Overview

Thank you for registering your interest in volunteering to be a Health Consumer Representative for Central Queensland Hospital and Health Service.

Our health consumer representatives have opportunities to participate in many aspects of our health service, from committee participation, consultation, focus groups or recruitment panels.

As a health consumer representative, you will need to be able volunteer a few hours of your time when needed for the engagement activity. Depending on what your interests are, you may be asked to complete an application and selection process to be part of some higher-level strategic committees.

Health consumer representatives have a unique and essential role in our health service. Without our health consumer representatives there is often no-one on a committee or interview panel who can represent a non-medical person and, in many cases, a broader community perspective. The health consumer has an important role in consumer and community engagement and will assist the health service with service planning and decision-making.

We are committed to a diverse and inclusive health consumer representation, reflective of our community.

You will not be required to have any specialist skills or health knowledge - let us focus on this.

To apply you need to live in the Central Queensland Hospital and Health Service region or Central West Hospital and Health Service region. Queensland Health staff members are unable to apply.

Please complete the following application form in full. Your personal details will remain confidential and will only be used for the purposes of consumer engagement activities co-ordinated by the health service. We will remove your details at any time on your request.
