CQ Health food and drink supply survey

Purpose: The purpose of this survey is to understand how staff and visitors to Central Queensland Health Service facilities feel about the food and beverage options available on site so we can make improvements to the products we have available. This is a survey being conducted by a project officer employed by Queensland Health.

Participation: You are invited to participate in this survey because you are working for or visiting our health service. Your involvement involves filling either an online or paper survey and it is estimated to take less than 10 minutes to complete. Your responses will be confidential and we do not collect identifying information such as your name, address, email address, etc. The survey questions will be about the food and drinks available for purchase at our health service facilities.

Please note this does not include food/drinks provided to patients by the hospital kitchen.

Your participation in this research study is voluntary. You may choose not to participate. If you decide to participate in this survey, you may choose not to continue completing the survey at any time. As this survey is anonymous, once you have submitted your survey, it will not be possible to withdraw it.

Confidentiality: All survey data is stored in a password protected electronic format and paper survey copies are destroyed after data is transferred to the electronic database. Surveys do not contain information that will personally identify you.

The results of this survey will be used to plan changes to the food and drinks available at Central Queensland Hospital and Health Service facilities. Results may be shared with staff and visitors and may be used in discussions at meetings/planning days at Central Queensland Hospital and Health Service facilities.
