CQ Youth Health Reference Committee application 2023

Closed 17 Apr 2023

Opened 27 Feb 2023

Overview

Thank you for applying for the role of Health Consumer Representative for CQ Youth Health Reference Committee.

If you need help completing this form, please call 49205776.

Volunteering as a Health Consumer Representative with us means you can participate in committees, focus groups, recruitment panels, review documentation and other engagement activities.

You will need to volunteer a few hours of your time, depending on the activity and what you are interested in.

To participate in some activities, such as recruitment and selection, you may be asked to complete some short online training courses.

You may be asked to complete a selection process if the activity is high-level like joining a strategic committee.

Without our health consumer representatives there is often no-one on a committee or interview panel who can represent a non-medical person or talk about lived experience and, in many cases, a broader community perspective.

A health consumer has an important role in consumer and community engagement and helps us to plan and make decisions.

We are committed to a diverse and inclusive health consumer representation, reflective of our community.

To apply you need to live in the Central Queensland Hospital and Health Service region. Queensland Health staff members are unable to apply.

Your personal details will remain confidential and will only be used for consumer engagement activities co-ordinated by the health service. We will not store or connect any of your personal information from engagement activities with personal medical record information and vice versa. For more information visit Health records and personal information | Queensland Health

Membership is free and is current for 12 months. To renew your membership, please complete this form.

We will remove your details at any time on your request.
